Are You Receiving Value in Your Treatment or Training?

The combination of chiropractic/manual therapy and massage therapy paired with smart training can make a profound impact on any musculoskeletal condition you may be dealing with. There's tremendous value in care and training that focuses on the goals and outcomes you care about. You place a value on your health or performance and you should receive services that deliver value.

The challenge is finding a chiropractor, therapist, or trainer who aligns with your values and the value that you place on your body and health. There's a spectrum on which these providers exist and it becomes your responsibility to do your homework. To make sure you find someone that can deliver the care, quality, results, and value that you're looking for.



You can go to 10 chiropractors and have 10 totally different experiences. You could see 10 personal trainers and have 10 totally different experiences. Yet all those experiences fall under the broad categories of 'chiropractic' or 'personal training'.

You don't know what you're getting into until you do some research and understand how someone practices or how they are going to approach your specific exercise program.

Some may argue one way may not be better than another way, but there are certainly more affective means of treatment and training that get better results.

There is a responsibility on the individual to find someone who will deliver that value. People often go through detailed vetting processes when it comes to a mechanic, electrician, plumber, doctor, etc. When it comes to your health, are you vetting the people you work with?

A big reason why we get a lot of athletes and individuals that prioritize their health is they do their homework. They want to understand how the body works and they are driven to optimize it. They search out the right person for the job. They want someone to deliver value in the care and the training that they are receiving because they realize they only have one body and they want to take care of it to the best of their ability.

Are you placing value on your health, fitness, or sport goals? Or are you just looking for the best price?

Exercise Hacks Ep. 12 - Do You Even Hoop?

In a Clinically Pressed podcast episode, I was presented the question of an influential purchase for under $100. I said the jump rope and Mark Bell's hip circle. If we take it a step further and go with a purchase under $10, I'd say the hula hoop.

Why the hula hoop?



Put your preconceived notions and research aside. The hula hoop may be one the most simple - yet challenging - tools to improve closed-chain mobility and motor control of the pelvis over the hips. The hips must move quickly in and out of rotation while the abdominal sling system of the body (see Exercise Hacks Ep. 11) works to move the pelvis. There's a balance of muscular stiffness for motor control as well as the rapid contraction and relaxation of musculature essential to athleticism. These same muscular qualities are critical in preventing back and SI joint pain.

The hula hoop first peaked my interest in a rehabilitation video featuring Jacob Chychrun of the Arizona Coyotes. There certainly seems value in its use as a rehab tool. In my opinion, there is value in its use for performance as well. From my experience, if used strategically, the hula hoop can offer similar benefits and muscular activation that the 'Stir the Pot' exercise offers. In the video we demonstrate the Stir the Pot and then hoop it up.

Rather than generating movement at the shoulders, we are generating movement at the hips while maintaining a stiffness through the spine to resist spinal motion. Hula hoop long enough, your obliques will be fried. That or your nervous system will be shot from the level of intramuscular coordination required in your functional core - the spine, shoulders and hips.

Dr. McGill has described the obliques as responsible for directing 'hoop stresses' in the body. It's a critical part of the Stir the Pot exercise. Maybe the hula hoop can be used to train the obliques how to better handle those hoop stresses as well. I'd really like to get Pavel Kolar's thoughts on this as well as other colleagues that may have some experience or insight.

Training to Maximize Athletic Potential

When it comes to athletes, critical developmental stages begin at an early age. As children mature, they progress through these important developmental stages during their growth and maturation process. If long-term athletic development is of any importance to the coach, parent, or athlete, specific aspects of athletic development must be addressed at appropriate time periods, otherwise the chances of the athlete reaching elite status is reduced.

The model used at Gallagher Performance began with a review of research and methods utilized in child and athletic development around the world. Through the review of current and past training methods used with elite athletes, it was concluded that to truly address athlete development, a new way of looking at how to properly structure “Strength and Conditioning” programs must be considered.
The reason is because early specialization in sport is becoming increasingly more common amongst children in the United States, and it’s not working. The rationale behind such a decision typically being if a child plays one sport, year round, they will be more advanced than their peers, more likely to be the ‘star’, get recruited, and/or possibly go on to make millions.

Recent research from UCLA reveals that early specialization in sport has very poor connection with young athletes achieving elite status. A survey of almost 300 NCAA Division I athletes found that 88% played two or three sports as children and 70% did not specialize in one sport until after the age of 12. These findings were already understood in former East Germany and USSR within their youth development programs.

Studies in former East Germany and USSR found that children who went through an early specialization program did have more immediate improvement in their performances. But these children also had their best performances between the ages of 15-16, had greater inconsistencies, many quit or ‘burnt out’ by the age 18, and they had greater rate of injuries because of forced adaptation compared to children who played multiple sports and specialized later in life.

Long-term athletic development is a process that occurs over many years. This is not an “8 week program”. Rather, it starts at an early age and continues on into adulthood. Long-term athletic development is about progressive development and must be approached accordingly. It is not simply a linear process, but is one that must be highly individualized to assist the athlete in reaching their full potential.

The greatest challenge to coaches, parents, and athletes is the understanding of how difficult this process is. Young athletes are continually dealing with massive changes in physical attributes, brain function, and sport skill acquisition. These all must be managed simultaneously while stressing the concepts of hard work in a positive environment.

Exercise Hacks Ep. 1 - The Dumbbell Row

In this video, we discuss the dumbbell row and how to improve both scapular stabilization and thoracic spine rotation during the exercise.

The dumbbell row is a fantastic exercise for building mass and strength in the upper back and arms. When done for reps (and no straps), it will build a solid grip. However, an often overlooked and undervalued aspect of the exercise is how awesome it can be in developing scapular, core and hip stability in the supporting limbs.

Some key points discussed in this video:

  • The traditional bench supported DB row utilizes hand support on the bench, which can be problematic for those with poor scapular stabilization.
  • Scapular stabilization on the support arm and be improved by elevating the bench to an incline position and supporting from the elbow instead of the hand.
  • With proper attention to posture during the support from the elbow will also improve thoracic spine rotation and mobilize the rib articulations during the exercise because of the increased spinal musculature involvement.
  • This can be a great variation of the DB row for those with shoulder pain, poor scapular stability or stiff upper back.

Exercise Hacks Ep. 2 - Scapular Upward Rotation

In this video, we build off our previous video on the dumbbell row and discuss scapular upward rotation. Scapular upward rotation is a critical component in overhead activities performed regularly in daily life, the gym or sport.

If your scapula cannot rotate properly as you reached overhead, it will play a role in shoulder pain and problems such as tendinitis, bursitis, or impingement syndromes. This exercise variation would be a great inclusion in the strengthening of the rotator cuff musculature and scapular stabilizers.

Some key points discussed in this video:

  • Most rehab exercises for scapular upward rotation will address bilateral movement at the same time. Training support on one limb while training movement on the other will enhance the quality expressed in stabilization.
  • Performed correctly, this exercise targets scapular stability on the support side and scapular upward rotation on the movement side while training proper core stabilization.
  • To heighten the global effect on the body, focus on creating a tripod with the support foot as well as hip external rotation to get the glutes involved.
  • This variation respects reflexive patterns of stabilization in order to improve movement quality and performance.

Exercise Hacks Ep. 4 - The Dumbbell Press

In this video, we discuss the dumbbell press and how the position of the wrist impacts pressing strength and shoulder stability.

Wrist position is often overlooked during the dumbbell press but is a critical part of ideal mechanics. The wrist should maintain a neutral position during the entirety of the lift. Often you'll see wrist extension and/or ulnar deviation as a compensation when the wrist falls out of neutral position. We discuss ulnar deviation and making sure to avoid this position as it can place too much stress on the wrist and feed into reducing pressing power.

As usual, the key is finding and maintaining optimal stabilization of the shoulder and upper extremity. Avoiding wrist extension and ulnar deviation will keep the wrist and shoulders more stable, healthier and stronger. The goal is to improve performance while reducing the risk of injury and pain developing in the shoulders, elbows or wrists.

Some key points discussed in this video:

  • What ulnar deviation looks like and how to avoid it while holding dumbbells.
  • How to press through the dumbbell with correct hand and wrist position.
  • How ulnar deviation will cause the shoulder to destabilize and fall into internal rotation. This isn't ideal for shoulder health and pressing mechanics.

Exercise Hacks Ep. 5 - Shoulder External Rotation

In this video, we discuss shoulder external rotation commonly performed on a cable column machine. This exercise is also frequently performed with the use of bands. Regardless of whether you use the cable column or bands, these pointers will still apply.

The quality of shoulder external rotation one can achieve plays a big role in overall shoulder health and upper body strength. Shoulder external rotation is dependent upon the quality of scapular stabilization one can achieve. That said, in order to improve shoulder external rotation, we can apply a hack to reflexively improve scapular stabilization.

By placing an object such as a yoga block, small exercise ball, or folded up towel or sweatshirt between your torso and elbow, we creating a fixed point to improve scapular stabilization. In doing this, it forces pure shoulder external rotation when the movement is performed well - making it very difficult to cheat!

Some key points discussed in this video:

  • The traditional cable column external rotation can be improved to heighten scapular stabilization and the demand on the external rotators/rotator cuff of the shoulder.
  • How to create a fixed point at the elbow to improve scapular stabilization by increased activation of the serrates anterior.
  • Avoiding ulnar deviation and maintaining a neutral wrist.
  • Increased global stabilization and muscular activation by setting the feet, hips, pelvis and spine into correct posture and utilize intra-abdominal pressure (IAP).

Exercise Hacks Ep. 6 - Deadlift Variations for a Functional Core

In this video we discuss two deadlift variations that will build rock solid functional core strength - the suitcase deadlift and pitchfork deadlift.

The suitcase deadlift and pitchfork deadlift are two deadlift variations that will expose energy leaks and strength imbalances real quick. Building a functional core often requires unconventional methods that go beyond aesthetically driven exercises. While unconventional these exercises force the lifter to generate total body tension to complete the lift - which makes them an awesome tool to teach lifters how to generate and maintain tension during squats, deadlifts or heavier training.
Each variation offers something unique.
1) The Suitcase Deadlift -  targets the lateral stabilization system or lateral line in the body as detailed in Anatomy Trains. The lateral line will create or resist lateral bend in the body and serves as a 'brake' for lateral and rotational movements of the trunk. The lateral line runs in balance on both the right and left side of the body from the skull down to the trunk and lateral aspect of the hips, thighs, calves and mid-point of the foot.
2) The Pitchfork Deadlift - this variation of the pitchfork deadlift also targets the lateral stabilization system while forcing the lifter to resist rotation. It really hammers the spiral line detailed by Thomas Myers in Anatomy Trains. The spiral line is responsible creating or resisting rotations in the body. The spiral line is a myofascial sling that includes musculature that begins in the neck down to the opposite shoulder and then to the same side hip, knee, and foot arch, then up the back of the body to rejoin the fascia on the skull. 
The bonus is both these exercises will reinforce patterns of stabilization in the body that are essential to movement.
These exercises are advanced and it is not recommended to go try them just to see if you can do them. They require a certain level of stability, coordination and strength to be able to do. Respect the process needed to progress to these exercises.

Pistol Squat or Skater Squat - Which is Better?

In this short video we discuss a commonly performed exercise in some group exercise or fitness classes as well as sport performance training programs - the pistol squat.

The pistol squat is a challenging exercise. For some it is a competitive exercise and one that they must train and improve.

However, what if you aren't training to compete in an event that includes pistol squats? What if you are training for general fitness or sport performance and want a better alternative? An alternative that will build great single-leg strength and control, and have better transfer to improving athleticism, speed, all while keeping the joints healthier?

Enter the skater squat.

You see for many people, the pistol squat can contribute to unnecessary compressive forces on the spine and hips that can lead to pain and movement intolerances. This is an unwanted result of training or exercise. Who wants to spend weeks working out only to takes weeks off due to pain or injury?

And these reasons are exactly why the skater squat becomes a better alternative to the pistol squat. You get a uniquely challenging single-leg exercise that builds strength, is more friendly to the spine and hips, and more closely mimics the dynamics of running, sprinting and skating.

The Most Effective Treatment for Shoulder Pain

Our approach to working with shoulder pain or injuries is very unique in that we really don’t work around shoulder injuries when properly indicated. The majority of the time, we don’t force someone to rest for several days while inflammation quiets down. Yes, it can be necessary in rare circumstances. However, rest and traditional methods of ice and anti-inflammatory meds are often over utilized when properly prescribed movement (i.e. exercise) is probably the better bet for improved recovery. At Gallagher Performance, we have a nasty habit of getting our athletes, clients and patients to work through pain and injuries by strategically incorporating exercise with specialized approaches tailored to find “the hardest thing they do well”. We want to find what they can be successful with while not posing any undue risk. And the best part? The system works.

When it comes to the shoulder, pain and injury is most commonly associated with poor shoulder function and faulty mechanics. And when I say most commonly, it’s not a stretch to say over 50% of shoulder injuries are due to these underlying problems. If muscles are unable to fire properly to provide ideal amounts of support (stability) and motion (mobility), then injury will occur regardless of how strong or flexible one is.
I have written extensively on the subject of the “hidden cause of injury” since the root cause of the overwhelming major of musculoskeletal injury is dysfunctional movement. Most doctors and trainers do not have the training or eye to look for dysfunctional movement and no amount of rest, ice, and Advil will ever provide the solution. No amount of passive modalities, taping, cupping, mobility drills, or stretching will correct dysfunctional movement. When it comes to shoulders, the solution to reducing shoulder pain and preventing injury is all about restoring proper rhythm and mechanics of the shoulder joint. This includes all it’s associated articulations – the glenohumeral and scapulothoracic. Basically we have to improve how both the “ball and socket” joint or “shoulder joint” (as most of us know it) and “shoulder blade” function together.

When it comes to improving the function of the shoulder, we’ll take an athlete or patient and have them perform variations of rows, pulls, presses, and ground-based movements with adequate loads to reinforce ideal body mechanics and correct dysfunctional movement or stability patterns. This is done through a combination of exercises, tempos, external stimuli, and progressions to essentially re-educate the nervous system. This is why it’s called neuromuscular re-education. We must teach the nervous system to do things better and break bad habits. Yes bad habits can be very challenging to break. Most people would rather pop a pill or rest until their pain disappears rather than break bad habits because breaking bad habits sounds like hard work. Popping pills and rest are great because they are easy and effective. But those who deal with chronic recurrences may want to think about a different strategy once their patience wears thin.
For example, some lifters have such poor thoracic spine posture and scapular dyskinesis that they may need 6 months+ of rehab and corrective work. But they have no interest in that. They have no interest in stepping back a little and refocusing their training for long-term development. They prefer to band-aid symptoms while they push their training and perpetuate the pain cycle.

The funny thing is breaking bad habits isn't as hard as most people think. If you work with the right person you’d be surprised what proper coaching, cueing, and exercise can do in a relatively short period of time. The reality is most people are highly resistant to breaking bad habits because of either ego or the unwillingness to take a step backward. Typically after a number of training or therapy sessions, the individual is able to perform any and all movements without pain and with improved shoulder mechanics. For those dealing with shoulder pain, improving their mechanics through sensory-motor training enables their nervous system to be re-educated. This re-education process is the most effective form of therapy and healing available. It cannot be understated that a critical component of this process is proper coaching and cueing. This is the responsibility of the coach or therapist. As much as proper coaching and cueing can be beneficial, improper coaching and cueing can prove to be very damaging. It is my opinion that many injuries occur because the athlete or patient had previously received very poor coaching or instruction.

This is exactly why performing exercise with proper technique is proving to be the single most effective form of therapy as it promotes almost immediate healing. If someone is having pain during a movement, they are doing something wrong. Correct them into the proper movement and watch their pain disappear. The key is re-educating the nervous system so movement becomes both therapeutic and performance-enhancing to one’s body rather than promoting pain and destroying joints.  This is truth when it comes to the shoulder joint as well as any joint within the human body.

Ultimate Runner's Guide to Injury Prevention

Running season is fast approaching. Spring and summer have a host of events from marathons, to half marathons, to triathlons, to various course races. Many have likely already started their training. And then of course we can’t forget those who will simply take up running in hopes of shedding unwanted body weight for the summer.

Whether you are taking up running to become healthier and lose weight, to qualify for Boston, or if you have your eyes set on crossing a race off your bucket list, your routine training will either build you toward your goal or you will be bogged down with nagging injury after nagging injury.

When you consider that 65-80% of runners will sustain an injury during the running season, clearly there is something that needs addressed to help runners cut down their chances of being sidelined or having recurrent issues during their training.

If there is one thing that most people know about me is that I’m not a distance runner. I’ll make that disclaimer up front. Never been a distance or endurance athlete and never will be. I live in the power-speed world of athletics. However, as a former hockey player and strength athlete, one of favorite past times and off-season training methods was (and still is) sprints.

Between my background as a chiropractic rehabilitation specialist as well as personal and professional experience in speed development, I’ve learned a thing or two about what it takes to build a body that is resilient to the demands of running/sprinting rather than breaking down. And at Gallagher Performance we have developed a reputation for not only building speed demons, but keeping their body healthy and ready in the process.

So what gives? Why is someone like me writing an article about running?

The name of the game in athletics is physical preparation and the same can be said of distance running. Unfortunately there seems to be a misunderstanding in that one only needs to run to be successful at running. While this may be true for some, there are numerous others who simply cannot solely rely on running in order to be prepared to run. Simply just running to be ready to run is an oversimplification of arguably the most complex human movement.

If that sounds ridiculous or confusing, let me explain my logic.

Most runners will eventually encounter their fair share of aches, pains, strains and overuse injuries. Plantar fasciitis, shin splints, tendonitis, stress fractures, runner’s knee, IT band syndrome and joint pain are common to the running community. Once training demands exceed what is one physically prepared for, this is where things start to go south.
These conditions may be present for a number of reasons, including any of the following:

  • Sharp increases in training volume
  • Foot wear
  • Gait mechanics
  • Strength deficits
  • Joint dysfunction or fixations
  • Improper motor control of lower extremities and/or torso
  • Overtraining
  • Inadequate physical preparation
This article is not intended to address training theory or programming as it relates to preparation for an endurance event, foot wear or gait mechanics. What I want to address is the reality that one must be physically prepared for a specific event and this requires that a runner must possess the necessary prerequisites in movement as it relates to running.
And no, being physically prepared doesn’t mean being fit or having a certain level of fitness. Being physically prepared for a distance running event goes far beyond one’s aerobic fitness.

To get my point across, allow me to use the analogy of intelligence. One can be intelligent yet being prepared for an exam in Civil War History is another issue. Now one may take that exam and it could go very well or horribly bad, but it doesn’t change the fact that the individual is still intelligent. What it means is they were either prepared or unprepared for that specific exam.

So while one may be “fit”, it does not mean they are physically prepared for a specific physical event. Even if one lifts weights, bikes, and jogs on a regular basis it doesn’t mean they are ready for a marathon. And most understand this, as they will specifically prepare for a marathon by training for it over a number of weeks.

But what is one to do to make sure their body is ready for the demands of running other than simply running? I mean that’s all one needs to do right? Just get out there and put in the miles right?

Yes, you will have to put your time in on the road or track. That’s a given. But there are also other considerations to make beyond the traditional means of endurance training (see this article here - 2 Common Misconceptions in Endurance Training).

The reality is running is tremendously demanding on the body and it’s even more so from a distance standpoint because of the need for far greater precision in running form, mechanics and motor control of the feet, ankles, hips and torso.

The need for strength and precision in movement control for the distance runner should make training strength and precision in movement control a high priority. This skill of awareness or proprioceptive ability can be trained through exercise. And this brings us to the heart of the article – ensuring you are physically prepared for running. Ensuring that your feet/ankles, hips and torso are more resilient against the cumulative physical demands of running.

Understand that I realize, like any competitive athlete, the cumulative trauma of the competitive season adds up and it is a challenge to stay 100% healthy. There are a number of variables that go into keeping one healthy. The hope is that through this article you gain an understanding of how training and maintaining certain physical abilities through specific exercises will not only help to offset what your body endures on the road, but make it more resilient as well.

The following exercises will serve to build the physical foundation that will help one stay healthier during training and the competitive running season, thus making sure your physical preparation meets or exceeds training demands.

While this list is far from comprehensive, it will serve as a general template to help runners to address basic physical prerequisites needed to stay healthy and train with minimal risk of setbacks. This is general template for physical preparation of a runner. Remember, like any athlete, physical preparation serves as your foundation as a runner. Take time to develop your physical preparation. Take time to develop your strength and movement control as it will allow you to get more out of training and keep your body healthy in the process.

Prevent Re-Injury with Integrated Training and Rehabilitation

The majority of us will not get through life without sustaining some degree of injury. The joints of the back, shoulder, hips, knees and ankles are all very common injury sites for not just athletes, but the general fitness population as well.

Most injuries that develop over time tend to have one thing in common, a breakdown in the human movement system. Meaning it could be that you are performing specific movements with sub-optimal technique or perhaps muscle imbalances are responsible for your symptom presentation. Regardless of the reason for injury, the goal is the same; to make movement more efficient to ensure that once training or competition resumes, the chance of re-injury is minimal.

Efficiency of movement is rarely a goal achieved in therapy. Incomplete rehabilitation in athletes and the general fitness population has lead to a re-injury epidemic. The problem is rooted in either the push to return athletes to the field as quickly as possible or rushing patients through the rehabilitative process.

With the ever changing landscaped of health insurance, the overwhelming majority of athletes and patients deal with increasing out-of-pocket expenses and limited number of therapy visits. Ultimately, many patients never complete their rehabilitation process.

This may be for a number of reasons, but in most cases athletes or patients are discharged once specific objective and ADL (activities of daily living) measures are satisfied. Sure you may have minimal to no pain, full range of motion and seemingly adequate strength resorted, and basic activities are easy to perform, but this does not ensure you are ready to resume training and competition.

And this is exactly where most get stuck.

They are lead to believe they are ready to resume sport training or their exercise program, but soon after resuming they realize they aren't as ready as they thought they were.

The transitional period between rehabilitation and performance-based training is the most critical period to ensure complete rehabilitation and that the transition back into training and competition carries minimal risk of re-injury.
Sadly, due to points made previously about the state of healthcare, many personal trainers and strength coaches are finishing off the rehab process.

Why do I say sadly?

Frankly, the majority of personal trainers aren’t educated enough to be overseeing such a delicate process, yet many position themselves as psedo-therapists. I’ve lost count of how many personal trainers I’ve seen giving “massage” or performing “joint mobilization” during their training sessions. They have no training or qualifications to perform such work and ultimately the person at most risk is the individual they are working on. Word to the wise: if your personal trainer is performing such work on you and has no license to perform such work, run the other way and seek out a qualified professional.
Within the fitness industry, there has been a large growth in facilities that blend rehabilitation with prevention strategies within strength and performance based training programs. Done well and overseen by qualified professionals, this is a great way to manage what is seen both in a rehab and training setting. This process should not be handled improperly. Implementing “corrective” or therapeutic exercises strategies into a performance-based training program should be lead by qualified professional(s). There used to be a gap between the professionals in the therapy and strength & performance world. Progressively though, that gap is slowly closing as more therapists crossover into the world of strength & conditioning.

Returning from injury isn’t and shouldn’t be a quick process. It’s far better to train smarter through the process. Improving on the function of the body while adding qualities such as endurance, strength, reactivity, power, etc. will help ensure successful outcomes. It’s less about isolation and more about training systematically to re-groove movement patterns. For anyone who has suffered an injury, they all want to get back to their previous level of function while also building the confidence they will not re-injure themselves. It can and will be a detailed process that involves rest, manual therapy directed at specific joints and soft tissues, as well proper exercise progressions. And yes, this means regressing, substituting, and even just slowing down exercises until they are owned.

Once movement and exercises are owned, it opens the door to further progressions in a performance-based setting to help ensure a more complete rehabilitation resulting in reduced risk of re-injury. This has become a huge part of what we do at Gallagher Performance as we successfully help our athletes and patients resume an active, pain-free lifestyle.

Summer Grind, Summer Blast

Lately it’s hard to find time to keep up with our blog. Life and business have a way of keeping you busy. Ignoring our social platforms may happen for a period of time, but we always revisit them. If there is one thing I’ve always hoped is that our blog would be informative, educational, and entertaining (at times).

The summer months bring on increased work load. Once May rolls around, we take it up a notch or two at Gallagher Performance. Summer is a grind, but it’s also a blast. We love the grind, love the process. With the volume of high school and collegiate athletes training for strength and performance, along with the patients we see ranging from acute care to rehabilitation to return to play, summer provides tremendous learning opportunities.

Reflecting back on the past several weeks, there are some friendly reminders and lessons learned or re-learn that I wanted to share:

  • Power-speed athletes thrive on power-speed drills and exercises. Just because one can squat or deadlift 500+ doesn’t mean they are explosive and fast. Yes athletes need strength and for many they will need a primary focus on strength training. However, those newly acquired strength levels must also be displayed in more power-speed dominant means such as sprints, jumps, throws as they have greater specificity to athletics than anything barbell related.
  • Athletes need to rapidly absorb force and rapidly generate force and do it on a level of unconscious activation. That brings me to another point of muscle activation. Muscle activation is a craze nowadays and rightly so. The overwhelming majority of the population will benefit tremendously from learning how to activate and integrate muscles such as their tibialis anterior, glutes, and scapular stabilizers to name a few. A lost art in muscle activation seems to be the use of isometrics. There is always an isometric contraction during the amortization phase of movement. Even during the most explosive movements, there is an isometric contraction. Isometrics are also awesome for reprogramming and generating a powerful mind-muscle connection, making isometrics a great tool for performance as well as rehabilitation. We have been utilizing a select few isometric drills for uprighting, motor control, and priming for improved  force/strength generation. In a relatively short period of time, they have more than demonstrated significant value.
  • There is a right way to go about training and a wrong way. The right way will always be dependent on the needs of individual and their specific goals. Don't get caught up in hype, trends, and empty promises. Trust the tradition. There is magic in the basics of the barbell, free weights, sprints, jumps, and bodyweight drills. They have stood the test of time. Fads and trends come and go, the basics remain. Using these exercises is one thing, understanding how to structure them in a training plan is another animal in itself. Find a trainer/coach that understands training specificity or else you are simply wasting your time and money.
  • We are problem solvers. Either as a clinician or trainer/coach, the heart of what we do is problem solving. Maybe it’s a matter of ability or effort, but clinicians or trainers either have the ability or they don’t. The ability to problem solve comes from knowledge and experience and even instinct. When it comes to effort, frankly some are just lazy and don’t care to think hard as it complicates their job. Whether it is listening to what a patient/athlete is telling you or just simply watching, you’ve got to process the source of the problem and how you’re going to solve it. When it comes to performance or rehabilitation, everything makes sense. If it is happening there is good reason for it. If we don’t understand it, it doesn’t make sense to us, but it always makes sense. Never dismiss a client or patient as not making sense. Make the effort to make a change. Change your perspective. Learn more.
  • We all need a coach. No one gets through life all on their own. We all have needed mentors and coaches at some point in our life. These may have been parents, family members, close friends, teacher, professors, bosses, etc. If we pursue something of significance, chances are someone helped us along the way. We need the help of others than have more knowledge, more experience, more accomplishments. I have had a number of mentors and coaches. For everything they did for me, I hope I can pass that on to those that I work with in the role as a coach.
  • Take time to get to know your clients and athletes. Show you care about them. We do more than just simply get kids bigger, stronger, or faster. We have an opportunity everyday to connect with our clients and athletes and hopefully make a positive impact. The reward goes far beyond cash flow. It’s about making a difference for the better.

Understanding Methods and Application in Training and Rehab

It seems, inevitably, we get weekly questions wanting our insights or thoughts on some training or rehabilitation method.

Regardless of the whether its the fitness industry or physical medicine profession, methods come and go all the time. Some interventions have staying power as they provide lasting results. Some trends are just a flash in the pan. The more recent or more intriguing the trend, the more it seems to generate questions.

When it comes to rehabilitation, this can be seen in a wide range of modalities and procedures from electromuscular stimulation (EMS) to low level laser therapy, machine-based exercises to the functional approach, stretching to myofascial release techniques, and kinesiotaping to cupping.

In the fitness and training industry, there is an equal (likely greater) amount of options and trends to get hung up on. From kettlebells to TRX, Curves to Crossfit, bodybuilding methods to Olympic weightlifting, and unstable surface training to over-speed training just to name a few.

While the question, "What do you think of....," may be seem to be a simple question in nature, it's a difficult question to answer without understanding the context of it's application.

Unless the application is understood, the results one gets from a specific method is left in question.

If you severely sprain an ankle during a basketball game, sure taping and bracing will help in the early stages of healing. As healing and rehabilitation progress, manual therapy and exercise begin to take more focus. Single-leg balance and sensory-motor stimulation have demonstrated successful application in the rehabilitation of certain injuries, such as ankle sprains. But if you get on a BOSU ball or unstable surface too soon - and you re-injure your ankle - is the problem the unstable surface or just poor application?

Similarly, there are many people who take on a fitness or sports training program but end up worn down and banged up because their application of certain principles is just wrong. This may be due to joining a group exercise class or working with a trainer that provides poor advice and application of training principles. Or it could be due to an individual attempting to structure their own exercise program without proper knowledge of training method applications.

We all could benefit from someone who we can trust for sound advice who it comes to applying the principles of rehabilitation or exercise. They will be able to inform you on what methods may be best for your specific goals and your unique individual considerations. You need someone who can help guide and educate you, who is able to critically think and problem solve. This is what the best trainers, coaches, and therapists are able to do for their clients and athletes.

What we do at Gallagher Performance is exactly that. We critically think and problem solve for our clients and patients. We aim to educate them and implement the most appropriate applications for their desired goals and outcomes.

Despite this, the reality is our philosophy, our approach, or our applications may not be for everyone. We won't sacrifice long-term sustainability for temporary results. We take pride in quality over quantity. We won't focus on the latest trends or what other people are doing. We aren’t concerned with this.

Our primary concern is offering the best training and therapy to the people we work with while educating them on understanding sound application so they are able to make informed decisions. And we will always educate, even if that means people have to hear the hard truth. But hopefully in hearing the hard truth, they learn lessons that provide better guidance in the pursuit of their goals.

Understanding the Benefits and Concerns of Youth Strength Training Programs

Benefits of Strength Training for Youth
Numerous studies have been published on the benefits of strength training in regards to overall fitness and health markers, muscular strength, injury reduction, sports performance enhancement, and confidence. Scientifically proven adaptations from strength training include increased neural drive, increased synchronization of motor units, and hypertrophy of skeletal muscle. These adaptations not only create a bigger, leaner, and stronger individual, but one who is able to express improved control and execution of complex sport skills while performing them at greater force and velocity outputs. So as a young athlete, if you have a desire to run faster, jump higher, or throw harder, you must first become stronger. Strength is the foundation on which all other physical abilities are built.

Experts also agree that there are many health benefits associated with strength training with research suggesting that strength training in youth can result in increased bone density, healthier body composition, and improved blood lipid profiles. Other benefits from participating in a strength training program also include reduced chance of injury during sport participation and increased self-esteem and confidence.

Now while to what degree strength and improvement in the weight room transfers into an athlete performing better on the field may be left in question, one thing that will always transfer to a competitive environment is confidence. I'm not talking having a massive ego or being cocky. Confident and cocky are completely different. Confidence is extremely important and strength has a unique way of improving confidence in children.

Concerns of Strength Training for Youth
Roundtable discussions including strength coaches, medical professionals, and researchers have focused on questions of concern pertaining to the strength and conditioning programs for young children. These concerns include injury rates, efficacy, and safety.

Among these experts, they have agreed on one common theme:

When a program is well supervised, form and technique are properly instructed, and the program is administered by someone who holds an appropriate certification, there should not be a concern for the child’s safety.

When it comes to weightlifting injuries, a large number of the reported injuries took place in a home gym or involved children who were unsupervised while they were lifting. In regards to minimizing risks in the weight room, many of the experts agreed that there should be an appropriate coach-to-athlete ratio (smaller ratios are ideal), proper education on strength training technique, and proper progressions for their training age.

Appropriate Age to Begin Strength Training
It is generally accepted that there is no specific age at which it is best to start a strength training program. However, it is recommended that children must be mature enough to accept and follow directions while also possessing an understanding of the risks and benefits associated with strength training. It is commonly accepted that if a child is participating in an organized sport, then this is an appropriate time period for them to begin a strength training program. Typically, for the majority of children this would approximately between the ages of 6-8.

Final Words
There are numerous benefits for youth to begin a strength and conditioning program. The program should be led by a qualified strength and conditioning professional and tailored to meet the needs in regards to age appropriate training, gender, and primary sport(s) of participation. Children should be willing and ready to follow instruction to ensure safety, quality training, and to meet their performance goals.

Attitude is Everything

Attitude.

We've all heard about it since we were young. We've heard it from parents, family, friends, teachers, and/or coaches who preached the importance of attitude.

Sometimes it takes a certain individual to have an impact in ways others cannot. Likely you have had someone in your life tell you the same thing you've heard countless times before, but for some reason it sticks. It sinks in. These people capture our respect. We feel they are deserving of our attention and we finally listen.

When it came to the importance of attitude, my Uncle Gene was just one person who made a big impact on my views. My uncle served in WWII and when he talked attitude, I listened.

Why?

It had everything to do with that fact that he embodied the message of "Attitude".

During his time serving in WWII, he was on a test flight near Foggia, Italy when a parachute flare bomb failed to release from his B-25. The bomb was hung near the rear hook on the shackle. With the safety wire pulled, the vane was able to spin freely. Realizing the danger of the situation, he climbed down into the bomb bay and, supporting himself with his elbows, he freed the bomb with his feet. For his actions he was awarded the Soldier's Medal for heroism. I have this medal in my bedroom to this day.

Now while his actions were awarded, it was his attitude in that moment that was the clear defining factor.

He was no more physically capable than any others aboard that plane. As a member of the Air Force, these men all had to be in top physical condition to serve our country. It wasn't that he was any bigger or stronger than the next guy. Simply put, in that moment he displayed resourcefulness and selfless courage to step into that situation. His attitude dictated his actions. Because of his attitude and actions, he received recognition for what he accomplished.

In a world where accomplishments matter, attitude matters most.

So what are some key characteristics of the attitudes or mindsets which promote success, be it in sport or in life?

JoAnn Dahlkoetter, Ph.D., author of Your Performing Edge and an internationally recognized sports psychologist, has identified characteristics that make a champion athlete that are able to be developed by anyone who wants to excel in sport:

  • Enthusiasm and Desire – Top athletes have a hunger, a fire inside which fuels their passion to achieve an important goal, regardless of their level of talent or ability.
  • Courage to Succeed - It takes courage to sacrifice, to work out when you’re tired, to seek out tough competition, to stick to your program, to test your limits, and to overcome obstacles.
  • Internal motivation and self-direction – Direction and drive need to come from within. The goals must be ones that you have chosen because that’s exactly what you want to be doing.
  • Commitment to Excellence – Elite athletes know that to excel at their sport, they must decide to make it a priority in their life. They make an honest effort each day to be the best at what they do. At some point you must say, I want to be really good at this; I want this to work.
  • Discipline, Consistency, Organization – Elite athletes love what they do and it is easier for them to maintain consistency in training and in competing. Regardless of personal problems, fatigue, or difficult circumstances, they can generate the optimal amount of excitement and energy to do their best.
  • Being focused and yet relaxed – Champions have the ability to maintain concentration for long periods of time. They can easily let go of distractions and take control of their attention. They don’t let emotions get the best of them and cause poor performance.
  • Ability to handle adversity – Top athletes know how to deal with difficult situations. Adversity builds character, but adversity also reveals character. When elite athletes know the odds are against them they embrace the chance to explore the outer limits of their potential. Rather than avoiding pressure, they feel challenged by it. They are calm and relaxed under fire. Setbacks become an opportunity for learning.
Many of the points simply brings the message back to attitude and the attitude one activity decides to embrace. Attitude is everything and I feel that Charles Swindoll puts this into perspective very nicely:
The longer I live, the more I realize the impact of attitude on life. Attitude, to me, is more important than facts. It is more important than the past, than education, than money, than circumstances, than failures, than successes, than what other people think or say or do. It is more important than appearance, giftedness, or skill. It will make or break a company, a church, a home. The remarkable thing is we have a choice every day regarding the attitudes we will embrace for that day. We cannot change our past...we cannot change the fact that people will act in a certain way. We cannot change the inevitable. The only thing we can do is play on the one string we have, and that is our attitude. I am convinced that life is 10% what happens to me and 90% how I react to it. And so it is with you...we are in charge of our Attitudes.

Low Back Pain Treatments That Just Won't Help

Back pain was believed to be a self limiting condition for the majority of individuals, meaning that the nature of back pain is that it would "run its course" and eventually pain would go away on its own.  Current research has demonstrated that this understanding of back pain is flawed; yet many clinicians still hold this belief.

In fact, 85% of people with a single episode of low back pain will likely experience future recurrences and 2-8% of those individuals will develop chronic back pain.

Chronic pain accounts for 75% of all healthcare costs related to low back pain, is second only to the common cold in missed days from work, and is the number one reason for workmen compensation claims.

Considering the burden chronic back pain places on healthcare resources, patient management appears to be an issue in need of addressing appropriately as many patients find it difficult to find effective treatment.

Low Back Treatments That Don't Help
Now many experts are questioning the model in which back pain is treated as a growing body of research suggests many common back pain ‘cures’ just don’t work. These include:

  • Ultrasound
  • TENS machines
  • Strong opium-type painkillers, such as diamorphine
  • Spinal injections
  • Spinal fusion
  • Disc replacement
Prescribing drugs or cutting people open when you don't know what's causing the pain is very unlikely to be successful in the long term. Surgery should only be used as a very last resort. There is a significant lack of efficacy for many treatments, but the deeper issue here seems to be that many healthcare providers have difficulty in accurately identifying the cause of ‘non-specific low back pain’.

Top spine expert, Professor Stuart McGill is the world’s leading spinal biomechanics researcher and has identified common reasons for back pain and the importance of exercise as an intervention for effective treatment.

In Dr. McGill's opinion, based on over 30 years of research, every case of back pain has a cause and the reason many treatments are ineffective is because they are used on a one-size fits-all approach, rarely targeting the underlying problem.
There are many possible causes for back pain, but you must first find the positions and stresses that trigger pain. As Dr. McGill says, finding these positions and stressors allows the provider to formulate a precise diagnosis and a roadmap to recovery. This roadmap is guided exercises that can correct the harmful patterns and build a stable, firm spine.

Addressing Misconceptions
The importance of a firm spine can come as a surprise to many, as the common perception is that your back must be flexible in order to be healthy and pain-free.

The spine must be firm and have strong muscles surrounding it to help transmit forces from the legs and the shoulders while minimizing the stress on the spine. When the muscles of the spine aren’t strong enough, micro-movements can occur that eventually can sensitize the spine and lead to a painful back.

If you have some movements, which are comfortable to build on, this opens the door to conservative management and recovery through guided exercise

However, recommendation of exercise without a clear understanding of movement intolerances and muscles that must be strengthened can also be harmful

Guided Exercise: The #1 Intervention for Low Back Pain
Exercise is essential both to protect and repair your back, but simply hitting the gym or doing Pilates or yoga without knowing the movement patterns that are generating your pain or the ones that will protect and build a pain-free back, has little chance of being effective.

Thanks to exercises specifically targeted at a patient’s problems, we are able to help educate them on proper posture and movement so they do not put damaging load on their spine. The guided exercise model is truly about identifying a patient's underlying back pain generators and educating them on not only how they can get out pain, but also what they can do to keep it from returning.

When you consider that 85% of individuals who have low back pain will experience future recurrences, there must be a priority placed on educating patient's about proper posture, movement, and exercises that build a firm, strong spine. Guided exercises are showing clear efficacy for use in patients with low back pain and should be a staple in their treatment plan.

Prior to attending Palmer, he earned a bachelor’s degree in Exercise & Sports Science from Ohio University.

After graduating from Palmer, Sean entered a residency program in Palmer College of Chiropractic’s Sports Injury & Rehabilitation Department. The residency is the only one of its kind within a chiropractic college in the United States. Under the direction of former Olympian, Dave Juehring DC, DACRB, CSCS and Ranier Pavlicek DC, ATC, DACRB, CSCS, the residency provided Sean the opportunity to further the development of clinical skills in the realm of diagnosis, treatment and management of sport-related injuries. During this time, he received extensive training in manual therapies and developmental stabilization methods influenced by the German and Czech rehabilitation schools.

Sean graduated from his residency and completed his board certification in 2012, making him one of a select few chiropractors in the country that have successfully completed a rehabilitation and sports-injury residency. He is a board certified rehabilitation specialist through the American Chiropractic Rehabilitation Board (ACRB) that abides by the standards set out by the National Commission for Certifying Agencies.

To compliment his clinical training and experience, Sean also serves as a Performance Coach with years of experience working with athletes of all abilities and is a certified Performance Enhancement Specialist (PES) through NASM. He is an accomplished athlete in the sports of ice hockey and Strongman. During his time at Ohio University, he was part of the 2004 ACHA D1 National Championship team. In 2001, he was named to the NHL’s Central Scouting Service “Top 10” High School players in the US and was ranked among the top players in North America (US and Canada). As a competitive amateur Strongman, he has won or placed in several NAS sanctioned competitions since 2010 and was a National qualifier in 2010, 2011 and 2012.

Stay Strong and Heal Faster While Injured

Injuries are a part of sport and life. It is an unfortunate reality and a lesson some encounter with greater frequency than others. I have had my fair share of injuries as well. The reason why I am writing this post is because of my most recent injury.

Over the past 14 weeks, I have been prepping for a strongman competition in Iowa on May 16. The training cycle had been going smoothly and I was feeling good heading into the final days before my taper. Four days ago, I pulled my left bicep during tire flips. The tire flip is one event that is notorious for causing bicep injuries due to the large amount of mechanical stress it places on the biceps. Fortunately, I did not suffer a complete tear, no surgery needed. However, competing is out of the question. When you are self-employed and your job requires the uses of your hands, there is no need for any further set backs.

For some, injuries mean down time from training. They see injuries as an obstacle. Not in my mind. An obstacle is what you see when you take your eyes off the goal. There are still ways to train around injuries. Sure, I will not be able to do anything stressful with my left arm for 3-6 weeks, but I can still get a powerful training stimulus from a incorporating squat and single-leg variations for lower body strength, jumps/bounds/hops for more intensive CNS stimulus, and training my non-injured arm to help maintain strength and speed recovery of my injured arm.

Wait….what? Training your non-injured arm helps to keep your injured arm strong and heal faster?
There is truth to that statement. The phenomenon I am referring to is known as “cross-education”. It is well established that to minimize the effects of detraining, performing single-side training with the non-injured limb (upper or lower body) will allow you to maintain strength and accelerate healing in the injured limb.

Cross-education occurs when you strength train a limb on one side of the body. The result is an increase in strength in the opposite limb on the other side of the body due to neural adaptations. Cross-education appears to be effective for all muscles and joints of the body, from shoulders and hips to ankles and wrists.

A study published in the Journal of Exercise Science and Fitness demonstrated that strength gains in the untrained limb are typically in the range of 5 – 25% depending on if that limb dominance. Strength gains average around 35 – 60% increase in the trained limb. Additionally, it appears that less range of motion will be lost in the injured limb due to the cross-education effect – another major benefit.

There are other studies on the subject of cross-education, but still cross-education is not completely understood. Strength gains in the injured limb are most likely due to neuromuscular adaptations and increased neural drive to the untrained muscle. A similar hypothesis is improved motor control because training the healthy limb results in recruitment of high-threshold motor units in both limbs. Keep in mind, there is no evidence of hypertrophy (muscle growth) or changes in muscle fiber types in the injured limb following single-side training.

Cross-education highlights the importance of single-limb exercises during training and rehabilitation from injury. Helping clients or athletes understand cross-education may encourage them to continue an exercise routine during time of injury, as it can help maintain strength and speed recovery. Cross-education is a perfect illustration of how one can turn a weakness into a strength through focused training efforts.
